Qt: Rally point key to "<ctrl><shift>r"

Description
Qt currently has "<shift>s" as the rally point key. Gtk-clients have "<ctrl><shift>r" as the rally point dialog opening. Change the Qt key to match, if possible (I haven't checked if there's any conflicts on Qt)

Also changes the Unit menu item un(S)entry all on tile shortcut from Ctrl/Meta Shift D to "shift+s", to match GTK client. See also Feature #1488 item 1.
